Dr. Craig Sorkin, DNP, APN

Dr. Craig Sorkin, DNP, APN is a board certified family nurse practitioner with extensive telemedicine experience. I have worked in primary care and GI and multiple virtual medicine care. I believe in the very core of our mission to provide you the best care at home. Whether that is hydration, care of a chronic condition or an acute care need, we are here for you.

Dr. Liliya Polyak, DNP, APN-C

Dr. Liliya Polyak, DNP, APN-C is a board certified adult gerontology, primary care NP with an extensive background in Emergency Medicine, as well as Sleep Medicine. Outside of the Emergency Room, she works very closely with patients that have a variety of sleep disorders, and creates a treatment plan to improve their energy levels and quality of life. She is also certified in Aesthetics and IV vitamin infusions. She is a sole believer that having proper hydration, and proper balance of vitamins and nutrients is essential towards obtaining weight loss, ideal sleep, high energy levels, and maintaining good health.

Dr Jonathan Mingoia FNP-C, DC

Dr Jonathan Mingoia FNP-C, DC is AANP board certified Nurse Practitioner, AAOPM board certified in pain management and performs various spinal injections. He believes when you feel your best you should look your best and offers medical assisted weight management.
